Price and Acknowledgement of Risk Waiver
Wilderness Martial Arts is offering class at the rate of $50.00 a month, no contracts to anyone ages 5 and up. Family* rate is set at $125.00, no contract. Students are required to read, fill out and sign that Acknowledgement of Risk waiver, which will be kept on file for one year. This will need to be renewed on the students anniverary. Without this form new students will not be allowed to particiate in Wilderness Martial Arts training.
* Family is defined and parent(s)/child(childern) unit of upto 5 individuals. Proof of relationship may be required. Arrangements can be made for family units of more than 5 individuals, but will be addressed on a case by case basis.

Fill out the Acknowledgement of Risk waiver and bring the printed copy to class. This must be fill out completely prior to participation in class.
